PRESCHOOL/KINDERGARTEN SCREENINGS!

WHEN: Monday, February 27^th-Thursday, March 1^st from 8:30 a.m.-5:45 p.m.

WHERE: Elementary/middle school library

FOR WHO: Incoming kindergartners for the 2012-2013 school year, incoming preschoolers for the 2012-2013 school year, and all 3 year olds.

WHAT: DIAL 4 screenings and kindergarten/preschool enrollment.

You MUST be screened if you plan to attend preschool for the 2012-2013 school year.

All 3 year olds and incoming preschoolers must bring in current immunization records, birth certificate, social security card, and proof of residency.

All kindergartners must bring in a current immunization record and proof of residency if currently enrolled in preschool this year. Otherwise, you must also bring in a birth certificate and social security card.
